Dr. Alex Lechtman, who practices at Visalia Medical Clinic's Aesthetic Center, will leave Thursday morning with a team of plastic surgeons with Interface — a volunteer group affiliated with the University of California, San Diego's School of Medicine — to La Piedad, Michoacan, to perform cleft lip and palate repairs, bone grafts and other procedures for nearly 100 people.

Makeshift hospital

Lechtman said operations will begin Friday morning and will continue Saturday inside a makeshift hospital set up by the Red Cross.

"We treat as many people as we can in two days," he said.

The hospital is set up in a building that allows for three operating rooms and six beds. Doctors will bring their own surgical instruments and medical supplies, he said.

Although the focus of the trip is cleft lip and palate operations for children, he said, other people from surrounding towns and states make the trip to the hospital for other cosmetic procedures, such as improving the appearance of burn scars.

"When the word comes out that we're coming, they start to line up at the hospital steps and wait for hours," he said. "These are folks in Mexico who have no other resources for care. This gives us an opportunity to help them."

Lechtman made his first trip to Mexico in 1990 while in medical school at UC San Diego. He then rejoined the volunteer medical mission in 1998 and has visited Mexico every other year to perform the surgeries.

The trips are paid for through community donations. The doctors also pitch in for travel and air costs, he said.

"This kind of operation is the reason why I got into plastic surgery," he said. "It's an amazing feeling to make this kind of difference in a patient's life. I feel very fortunate to help."

Q: Why doesn't my belly get flat again after kids? I exercise, eat right and nothing seems to work.

A: Many women who have had children are surprised that their tummies are no longer flat regardless of how thin they become and how much they work out. This is because the abdominal muscles spread apart as the baby grows and often never go back together. The thinner tissue between the muscles is called a rectus diastasis, and this problem is sometimes accompanied by a hernia around the belly button. If the diastasis is very mild, abdominal strengthening may restore it, but if it is wider some women may opt for a surgical repair called a tummy tuck. The hernia can be repaired at the same time.

Q: Is there a way to avoid a large scar across the breast and preserve the nipple when having breast reconstruction after cancer surgery?

A: Many women are advised that they need to have their breasts removed because of cancer, precancer or a genetic predisposition for cancer. Some of these women may be candidates for a newer technique of skin and nipple preservation and single-stage reconstruction, without requiring the usual expanders or nipple reconstruction. The results are cosmetically superior, the post-operative pain is reduced and the recovery time shorter. Women should ask their surgeons if they are candidates for the procedure.

Q: If either I or someone in my family lands in the E.R. because of a non-life-threatening laceration on the face, at what point do I ask for a plastic surgeon?

A: Most emergency room physicians have extensive training and experience in dealing with lacerations. Often, people will ask for a plastic surgeon on call when the emergency room physician is perfectly capable of handling the situation, and may in fact have more experience than the surgeon on call. As a rule of thumb, lacerations involving the eyelids, lip margins, ears and nostrils can all be indications to call a plastic surgeon because they involve complex cartilage or muscle repairs. It is important to feel confident in the ability of your physician at all times, so if the emergency room physician is too busy or does not appear to be confident in his or her ability to treat the injury, it is appropriate to ask them to call a specialist.

Boxer banned for breast implants

A female boxer has been banned from the ring – and it’s not because of her gender.

The former model says it’s because of her breast implants, London’s Daily Mail reported.

Sarah Blewden, 25, of Poole, Dorset in England, told the newspaper that boxing officials will no longer allow her to fight because they fear repeated blows to the chests would ruin her false breasts and “damage her breast tissue.”

Blewden began boxing two years ago as a way to keep in shape, but decided she had a real talent for it, so she applied to the Amateur Boxing Association.

When the Association found out she had the implant surgery in 2003, officials denied her request to fight.

The Association’s medical expert told Blewden she was at risk of distortion, and the breast tissue’s blood cells could be damaged.

Blewden appealed, arguing she would wear a breast protector, but was denied.

“They are gel implants and not liquid, so they won’t burst,” she said. “And if I suffer capsular contracture and need to pay for corrective surgery, then that is my decision.”

Guys have always had their own set of grooming tools, but now many men are embracing the idea of wearing makeup, and brands are responding with primping products just for them.

Sheyla Hershey, 28, housewife and model has undergone nine ops to get her massive 38KKK breasts-which sets the world record for the Largest breast implants .

Sheyla Hershey traveled to Houston eight times to have her breast enlarged to a staggering 34 FFF. "To me, big is beautiful. I don’t think I have anything to worry about," she said. (enlarge photo)

According to NBC New York, that was not good enough for Hershey so when doctors in Texas refused to augment her already ginormous breast any bigger she went to went to Brazil for her dream--- a 38KKK bust, and a new world record for largest breasts implants.

And even though medics have warned that her breasts are in danger of exploding, she does not seem to care.

Her British ex-boyfriend started paying for her plastic surgery, but she left him after he begged her to stop. She said: “I loved him very much but I had to leave him to follow my dream.”

Why go to such extremes? Sheyla Hershey said she didn't start out with the goal of having the largest breasts in the world.
"I started because I had a picture of Dolly Parton in my head since I was 9, and I just love the way her body is," Hershey said. "I thought, 'I want to have that body.'"

The Brazilian native has lived in the U.S. for two years. She had her first breast augmentation in 2001 and 10 more since -- all in her home country. When she reached her previous FFF size, she spoke with a U.S. doctor about going bigger. "He said that's crazy," she said. "He didn't think this was attractive at all."
Plus, she said, she was already over the maximum allowable size in the U.S. So she headed back to Brazil. "I had a dream, and I didn't stay home and dream of my dream -- I just went for it," she said.

The U.S. Food and Drug Administration has not approved implants larger than 800 ccs, FDA spokeswoman Siobhan DeLancey said. That's .85 of a quart per implant. Hershey's smaller implants at size FFF were already around 2 quarts per breast, Fox News reported.

"Implants that are larger than 350 ccs are too large for many women," DeLancey said. They can increase the risk of developing complications such as extrusion (popping out), hematoma and implant folds (when the implant folds in on itself).

...Can you run with them? According to plastic surgeon blog The Plasticologist, "Running typically subjects the body to the shock of the feet hitting the ground - this force is transmitted through the knees, spine and pelvis. Large implants may increase this force over time as you as supporting additional weight, especially in the shoulders and spine."

"Getting breast implants which are too big can get in the way of efficient arm movement. Part of running is the forward and back movement of both arms as we run. Typically this can be an issue with more established runners as they' re patterns are more difficult to change", according to the Plasticologist.

Large breast implants can also hinder arm movement.

Having such large implants is tough on Hershey's frame. She performs special exercises regularly and gets massages one a week. "I have good care for my back," she said. "If you don't have care for it, you're going to have pain."

Sheyla Hershey said her husband likes how her breasts look, but that he's concerned about her health and wants her to cease with the implants. She said doesn't plan to keep the implants in long-term because of the health difficulties.

* An astounding 3 percent (nearly 3.5-million working women) say they've already had a cosmetic procedure to increase their perceived value in the workplace.

* 73 percent (almost three out of four or, 84-million working women) believe, particularly in these challenging economic times, appearance and youthful looks play a part in getting hired, getting a promotion, or getting new clients.

* 80 percent (four out of five or 92-million working women) think having cosmetic medical procedures can boost a person's confidence.

Insurance Broker Janice Axelrod, a baby-boomer, recently had a chemical peel and fat transfers from her abdomen to her face. "Time has given me the professional knowledge. But time can take away the youthful sparkle of my appearance if I let it. When you look good, you feel confident. That gives me a competitive edge and something my clients have come to expect from me," says Axelrod.

ASPS Member Surgeon Loren Schechter, MD performed Axelrod's cosmetic procedures at Advocate Lutheran General Hospital in Park Ridge, Illinois. He says "Not only do the women believe youthful looks help in the workplace... they're acting on that belief."

John Canady, MD, president of ASPS says, "Consumers need to remember that while cosmetic procedures might help them in the job market, they're still medical procedures." Canady says women still need to proceed with caution.

Summer is right around the corner and so is the dreaded bikini. You may be getting into shape or just deciding what you are going to do to rid yourself of unwanted hair. Whatever the case, there are various methods and products available on the market to help with your bikini line, underarms, legs, and anywhere else where hair might be inhibiting your summer fun.

Razors have been my only means for shaving my legs, but years ago, I was getting cuts frequently with my cheap, disposable razors. One day, I decided to use my boyfriends Gillette Mach3 Turbo and I haven’t been back since. With a lubricating strip and enhanced micro fins, this three-blade razor promises a close, comfortable shave without nicks and is safe to use anywhere.

For smoother results, I recommend products by Nair. Nair promises better results that last days longer than shaving and instead of cutting the hair like razors, Nair removes hair from below the surface for a softer touch. I only usually use Nair for my bikini line so “bikini cream” is just right producing a stubble-free area that doesn’t grow back for about three days. Nair’s bikini cream is available at Wal-mart for about $4 and also comes in an easier glide-on version. If you can’t stand the smell of original Nair products, look for the new lotions that come in fresh scents like cucumber melon and cocoa butter.

What about wax? If you’re brave enough to try at-home wax, try Bliss’s Poetic Cosmetic Waxing Kit. I found that this kit is actually organic, containing oils and infused with aromatherapy. Bliss’s kit includes the wax, large and small spatulas, cleanser, lotion, and a cute little mug. Available at Sephora for $40.

When I get sick of maintaining my bikini line, I go for the Brazilian bikini wax. What it is: very thorough waxing that removes every last bit of hair except a thin landing strip. Yes, it is painful, but the results are amazing and can last up to 4 weeks. Available at Spa Sydell for $65 plus tip. If the Brazilian wax isn’t your cup of tea, try a regular bikini wax, which can be just as painful, but produces the same results, for about $35 at any spa.

If you’re looking for a more extreme result for hair removal that actually lasts, look into laser hair removal. Laser hair removal uses a pulsating laser light to remove unwanted hair pretty much anywhere including the upper lip, bikini line, back, legs, and underarms. Although laser hair removal may require more than one treatment to get rid of all of the hair, the procedure produces long-term and even permanent results. Averaging about $388 per treatment, laser hair removal can be performed by a certified dermatologists or dermatologic surgeon.

Any way you chose to remove your unwanted hair, make sure you use the right products to soothe your skin and prevent bumps, burn, and especially ingrown hairs. Try Tend Skin, a product available from Sephora for about $25. Useful for both men and women, Tend Skin is useful for any cosmetic problem related to hair removal.
C.O. Bigelow also offers a light after-shave lotion infused with eucalyptus oil and antioxidant grape seed oil that soothes skin from irritation. This product is fragrance-free and dermatologist tested for people with sensitive skin and can be found for $24 at Bath and Body Works.

If anything, make sure to stay attentive to your skin to prevent serious razor burn, bumps, nicks, or ingrown hairs. Doing this will keep your newly smooth skin healthy.

Tummy tucks for men?

Women aren’t the only ones who care about their bodies. Though men are less open about it they, too, want to look and feel their best. Most men develop loose skin after a significant weight loss or gastric bypass surgery. Aging, too, can cause the skin to lose its elasticity, and result in sagging skin.

Tummy tuck surgery, formally known as abdominoplasty, can be performed as a full, partial or extended abdominoplasty. The closer you are to your ideal weight, the better results you can expect. A cosmetic surgeon who is certified by the American Board of Plastic Surgery should perform your abdominoplasty.

A full tummy tuck involves incisions from the pubic area up to each hip, and vertically to, and around the belly button. The skin flaps are separated from the muscle, and then the muscle connective tissue is sutured to bring it closer together, flattening and tightening the muscles. Liposuction may be done at this point if there is excess fat. The skin flaps are then pulled snugly to fit the abdomen, trimmed to fit around the navel and to remove excess skin, and sutured. Lastly, drains are placed to reduce fluid buildup, and will remain in place for a few weeks, until swelling goes down and healing is nearly complete. Surgery can take up to five hours, and is done under general anesthesia.

Partial tummy tuck is similar to its full counterpart, but involves only the abdominal area below the level of the navel. The incision above the pubic bone is shorter, not extending all the way to the hip, and there is no need for the vertical incision, as the skin above the navel will not be displaced. The procedure is similar–liposuction may be done if there is excessive fat, and the lower abdominal muscles tightened by suturing. The skin is pulled down and trimmed off, then sutured closed. Drains are needed to reduce fluid buildup, which remain in place until swelling begins to resolve. Surgery takes about two hours, and is done, usually, under general anesthesia. ASPS has animated both procedures to give you a better understanding.

An extended tummy tuck reaches beyond the abdomen to the sides, above the hip, to remove those precious love handles. The incision extends beyond the hip, and an additional procedure, liposuction, can be done to remove excess fat. Aside from including an area that extends to the sides of the abdomen, the procedure is not much different from a full-abdominoplasty.

Treatment: I chose the Aqua detox scrub as my New Years resolution was to look after my skin more. I was entrusted with a very sweet therapist who delicately buffed my body all over with a brush and then applied a layer of Champneys aqua therapy scrub, rubbing in the salts to remove any dead skin cells and smoothen out the skins surface. I was then asked to shower off the remaining salts. Love Your Body

According to a global study conducted by the National Organization for Women (NOW), in 1998, a mere 2 percent of women considered themselves beautiful. This startling statistic, along with others just as sad, sparked the NOW organization to start the Love Your Body Campaign—an attempt to redefine beauty by making the term less exclusive and narrow. By now, most of us probably associate this movement with Dove and their allied Campaign for Real Beauty.

This advertising scheme, which has received tons of press, both negative and positive, uses “real” women in their ads— a mixture of races, pant sizes and facial structures. Proponents hail the campaign as revolutionary and empowering, while critics remind audiences that all advertising schemes play on insecurities. After all, if a person is gorgeous and perfect, they wouldn’t need to buy any beauty care products at all.

I’ve always admired Dove’s ads. Despite all the inherent flaws of the campaign, I like seeing women with “imperfections” confident and loving what their mama gave ’em. I like the variety and the statement they are making. Whether they airbrush and touch up their own ads or not (which they very well might), they are the company that has actively exposed this overused technique to the public, reminding us that even “beautiful people” aren’t as beautiful as they look in magazines. To be honest though, I never felt very inspired myself. Seeing those women feel confident never actually raised my level of self-esteem. Maybe the advertising industry needs a bit more rattling before Beauty can fall off its pedestal.

I have, however, had what I call my own “Love Your Body Campaign” moment.

My grandmother is beautiful— she always has been. Year after year, I visit her, and I am always struck by how great she looks; she has bright greenish-golden eyes, a head of thick white hair, tanned skin and on top of it all, she is a very sharp dresser. Her perceptiveness and humor only add to her loveliness.

This year was no different, and again I was happy to see her looking great. But a few days after Christmas, we were standing in the hall in front of a large mirror, and she was trying on a new jacket. She was looking at herself in the mirror and dancing around while my mom and sister complimented her reflection, and I stood aside looking at her.

I became suddenly aware of her wrinkles. It sounds terrible, but I did. I couldn’t help but compare the firmness of my skin and the solidity of my structure with her smallness. I remembered that she can no longer play tennis because her joints are weak. Her knuckles are thick and swollen due to age and her nails are brittle. My body, in comparison, screams youth and vigor. In that moment, I loved my body, probably more than I ever have before. For the first time, I appreciated why older women are always telling me that I look great, and why they laugh when I immediately say, “Oh come on” and point to my thighs or a zit.

I feel guilty using my grandmother’s flaws to recognize my own beauty, and I don’t mean to imply that older women can’t be beautiful, because I still see all the loveliness in her that I have seen every year. What I am saying, however, is that youth is an incredible thing. Our bodies have the most potential right now, and to spend it hating them, or wishing they looked different, is an awful waste of that opportunity.

Destin is becoming to the Emerald Coast what Beverly Hills is to California.

Two local plastic surgeons will be participating in a new breast implant study along side Dr. Robert Rey of E!'s "Dr. 90210."

Dr. William Burden and Dr. Scott Ennis of Destin Plastic Surgery are among only 30 plastic surgeons who were chosen from the United States to be included in the study.

Mentor Corporation, a leader in breast implant manufacturing, has created a new adjustable silicone gel implant that is now in clinical trial.

Mentor chose the 30 surgeons based on their experience with silicone implants and credentials, according to the news release. Burden and Ennis are the only plastic surgeons in Northwest Florida, Alabama and Mississippi invited to participate.

"It is an honor to be among the nation's top plastic surgeons leading our field in the newest and latest technique and product," said Burden in the news release.

The implant is currently being used in Europe and Mentor is seeking FDA approval in the United States.

"We are pleased that our extensive surgical experience has garnered us this opportunity. We feel it is a great boost to have Destin included with larger destinations including Beverly Hills, New York and Las Vegas," Ennis said in the release.

Allergan stays beautiful amidst an ugly economy

The faltering economy has reduced the spending capacity of the consumer to indulge in cosmetics. This results to lower profit for most cosmetic companies, save for Allergan—the renowned manufacturer/distributor of Botox.

In the upcoming months of 2009, Allergan Botox will be getting competition from a rival medical manufacturer. Reloxin—a competitor of Allergan Botox—is now in the pending stages of approval of the Food and Drug Administration (FDA). But that’s the least of Allergan’s problems, because the company still has to worry about staying afloat amidst the economic crisis.

Experts anticipate a 10 percent decrease in Botox sales and breast-implant sales. However, Allergan has invested in many areas of the cosmetic industry—a move that secures its place in the market.

Allergan has invested in the eye care sales, which can curdle losses to make the company more stable. The company also celebrated the FDA-approval of a new product: Latisse. It’s a topical solution, which is used to grow eyelashes. Also, experts say that once prices and consumer demand have stabilized, there’s a big door for respectable profit.

Other cosmetic companies that are fairing relatively compared to others are: Avon, Estée Lauder, Medicis Pharmaceutical, and Bare Essentials.

Cosmetic surgery not just for Beverly Hills anymore

With the new year, many will look for ways to improve upon themselves. For some, it might be setting exercise goals or adopting healthy eating habits. For others, improving upon one’s self-image might mean making cosmetic improvements, like an eyelift or tummy tuck.

Dr. Loucinda Dampier has opened the L.R. Dampier Cosmetic Surgery Center in Maryville to offer cosmetic surgery in Blount County. Dampier’s office is located at 103 Station Drive at High Street.

Cosmetic surgery isn’t just for people who live in Beverly Hills, says Dr. Dampier. She said it is common for patients to be regular, working people who want to make an investment in themselves. Material possessions won’t last forever, but a person has to live with their body, Dampier said.

“I’m looking to help people feel better about themselves,” Dampier said. “To help them feel more comfortable in their own skin.”

According to Dampier, a lot of her patients are women who, after childbearing years, choose to get a tummy tuck and breast lift or implants. She said this if often referred to as the “mommy makeover.”

“I can’t turn anyone into someone else,” Dampier said, but she can repair some of the effects of childbirth, breast-feeding and aging. She said a lot of women want to remove stretch marks, tighten the stomach and lift the breasts to help get their body to what it looked like before they had children.

Dampier said a trend she saw in Las Vegas was buttock augmentation. She said it is a procedure that removes fat through liposuction, usually from the stomach region, and injects it into the buttocks.

According to Dampier, another popular trend is the facial rejuvenation through procedures using filler products such as Botox. Dampier said this in-office procedure is interactive for the patient. They can see in the mirror the changes as the procedure is done. It is common for the filler to last at least six months to one year.

Dampier said that men are becoming more interested in filler products to help get rid of crow’s feet, frown lines and forehead wrinkles. Also, a popular trend in cosmetic procedures for men is eyelid rejuvenation.

Dampier, who grew up in South Georgia, said she chose to live and practice in this area for several reasons.

“We chose Maryville because it didn’t have a cosmetic surgeon in the community,” Dampier said. Also, Dampier said she was attracted to the area because of the quality of life. Growing up, her family vacationed in the Smoky Mountains and that is when she fell in love with the area.

Dampier was a young teenager is when she decided on her profession as a cosmetic surgeon.

“I’ve know since I was 14 years old what I wanted to do,” Dampier said. She said she had to have several moles removed and weekly visits to the plastic surgeon is where she became fascinated with the field of cosmetic surgery. She had the opportunity to see the operating facility and became intrigued by the changes to the patients after their surgeries.

Dampier was so driven by her goal to become a surgeon, she skipped the last year of high school so she could graduate early and go to the University of Georgia. She received her college degree at 20 years old. Then, Dampier graduated from Mercer University School of Medicine in Georgia and completed her surgery residency in Jacksonville, Florida, becoming one of the youngest surgeons to complete board certification. After board certification, Dampier became a Cosmetic Surgeon Fellow at the Plastic Surgery Institute in Las Vegas.

Dampier said there is a difference between a true cosmetic surgeon and a plastic surgeon. Dampier said she chose to become a cosmetic surgeon because she wanted to focus solely on aesthetic surgery.

“Cosmetic surgeons train at private or group practice surgery centers focusing primarily on aesthetic cosmetic surgery,” she said. “Plastic surgeons receive their training at an advanced residency program through an academic center. Plastic surgeons receive a broader range of experience like burn and trauma reconstruction as well as some aesthetic surgery.”

Dampier said it is very rewarding to see patients feel emotionally better after cosmetic surgery. “I really like the confidence it gives my patients,” Dampier said.

Botox, Cosmetic Procedures Increased For Inauguration Festivities

Cosmetic dermatologists and skin experts in our nation’s capitol and other cities note that even faced with a poor economic situation, requests for Botox and microdermabrasion have increased as people prepare for inaugural parties.

"We have been absolutely swamped since the election with people desiring rejuvenation procedures for the upcoming inauguration," cosmetic dermatologist Tina Alster told USA Today.

"My normal load for cosmetic procedures has doubled, except for hyaluronic acid fillers — Perlane and Restylane — which have almost tripled," added cosmetic and laser surgeon Hema Sundaram, located in Washington, D.C.

Although a 2008 survey released by The American Academy of Cosmetic Surgery exposed that 96% of cosmetic surgeons are worried about the economic situation and 79% are hurting, patients like interior designer Jan Lynch, 59, says that a little depression can't stop her from looking her best on January 20th.

"My motivation is to look the very best I can for such a historical event. I've flown in to D.C. from California for this," says Lynch.

Sundaram has male patients, but the majority of her patients are ladies over 35, such as socialites, politicians, and others present at inaugural events.

Journalists, politicians, lobbyists and lawyers all pay for the quick beauty procedures. "They have all come in at once," she added.

Washington, D.C. is not the only hot spot for inauguration beauty procedures.

Cosmetic dermatologist Jeffrey Dover, of Chestnut Hill, Mass., notes that patients are "queuing up" at his office for parties. "They're asking for no-downtime procedures to look their best for the festivities."

Leslie Baumann, head of the Cosmetic Medicine and Research Institute at the University of Miami, told U.S.A. Today that, "We were swamped this week. Some were going to the inauguration in D.C. But I saw many going to inauguration parties in Miami."

New York City cosmetic dermatologist Cheryl Karcher has seen a large increase in inauguration-related procedures at her office recently. Karcher recently worked on several ladies planning to travel together to Washington, D.C. "That was fun. They all got peels, Botox, fillers."

The recession is to blame for everything going wrong right now—the rise in foreclosures, the increase in unemployment and the sharp decline in the number of people who want size DD breasts. Plastic surgery numbers are steadily slipping here in the U.S. Americans, who once spent $12 billion a year on cosmetic surgery, are now tightening their budget according to the American Society of Plastic Surgeons. There’s been a 62% decrease in plastic surgery from 2007-2008. Consumers are hesitating now more than ever before to commit to having the pricey elective procedures done. Doctors in Orange County, Calif., the silicone capital of the world, have reported a 30 to 40 percent decline in business. And breast implant manufacturer Mentor Corp.’s sales have decreased 5 percent during the three months ending Sept. 26 compared to the same period last year. But one country isn’t experiencing a decrease in their numbers. Find out which one, after the jump.

A sagging economy doesn’t mean the women of Venezuela walk around with any part of their bodies sagging. An estimated 30 percent of Venezuelan women ages 18 to 40 have had breast implants, according to the Venezuelan Society of Plastic Surgeons. Around 30,000 to 40,000 women a year get implants. Venezuelan men also are getting into the act, having more and more nose jobs, liposuction and shots of Botox. Men now account for 10 to 20 percent of plastic surgeries in Venezuela. This is not surprising in the South American country. Renowned Venezuelan plastic surgeon, Dr. Peter Rome, said in an interview, “In Venezuela, a woman may not have enough money for her home, but she will spend to keep up her appearance with plastic surgery and new clothes. Allergan Breast Implants FDA Approved

Allergan® Silicone-Filled Breast Implants are FDA approved.

With the recent approval of silicone gel-filled breast implants for breast augmentation and breast reconstruction, physicians and patients can be assured of the safety of silicone. This confirmation is based on extensive preclinical testing, four years of data on 715 women from Allergan's Core Clinical Study and a European study that evaluated implant rupture prevalence rates beyond 10 years. In addition, countless published, peer-reviewed studies and research support the safety of silicone gel-filled breast implants.

A study by the US Government looks at silicone gel-filled breast implants and confirms findings.

In 1997, the Department of Health and Human Services began one of the most extensive research studies in medical history by appointing the Institute of Medicine of the National Academy of Science (IOM) to examine potential complications during or after surgery.

The IOM consisted of a 13-member volunteer committee, including six women. The committee was composed of members of the medical, scientific and educational communities with experience in radiology, women's health, neurology, oncology, silicone chemistry, rheumatology, immunology, epidemiology, internal medicine and plastic surgery. No IOM members had on-going relationships or conflicts of interest related to any implant lawsuits.

The result: After reviewing years of evidence and research concerning silicone gel-filled breast implants, the IOM found that "Evidence suggests diseases or conditions such as connective tissue diseases, cancer, neurological diseases or other systemic complaints or conditions are no more common in women with breast implants than in women without implants."

Furthermore, a review of research and medical studies shows:

- Extensive studies, including a report by the Institute of Medicine, conclude that breast cancer is no more common in women with implants than in those without implants

- The American Academy of Pediatrics concluded in September 2001 “The Committee on Drugs does not feel that the evidence currently justifies classifying silicone implants as a contraindication to breastfeeding.”

- Epidemiological investigations have not found any increased risk of adverse health outcomes… in children born to women with breast implants

Silicone gel-filled breast implants are arguably the most studied medical devices and decades of research have evaluated their safety and effectiveness.

The Allergan Core Clinical Study

Allergan's Core Clinical Study is an ongoing 10-year study of 940 women who had breast augmentation, reconstruction or revision of a previous surgery between 1999 and 2000. Safety and effectiveness is evaluated through patient follow-up at zero to four weeks, six months, 12 months, 24 months, and annually through 10 years. Safety is assessed by complications, such as implant rupture, capsular contracture and re-operation. Benefit (effectiveness) is assessed by patient satisfaction and measures of body image/esteem and self-esteem.

The result: The FDA has evaluated Allergan's Core Clinical Study data at four years and determined that INAMED® Silicone-Filled Breast Implants are safe and effective for use in breast augmentation and breast reconstruction.

The Allergan Adjunct Clinical Study

The Allergan Adjunct Clinical Study enrollment was limited to reconstruction and revision surgery patients who met certain inclusion criteria. Between the years of 1998 and 2006, when enrollment in this study was terminated, over 80,000 silicone gel-filled breast implants were implanted in more than 50,000 women. Safety data is collected at one, three and five years and is used as supplemental data to support the safety of INAMED® Silicone-Filled Breast Implants.

Studies show silicone gel-filled breast implants are safe, but that doesn't make them right for everybody.

Silicone gel-filled breast implants are not lifetime devices. It is possible at some point in your lifetime that the implant(s) would need to be removed or replaced. To ensure that you achieve your optimal results safely, patients should be aware that you should not have breast implant surgery if you:

- Have existing malignant or pre-malignant cancer of the breast and have not been successfully treated

- Have an active infection anywhere in the body

- Are currently pregnant or nursing

You should also know that silicone gel-filled breast implants have not been clinically tested in women with:

- Autoimmune diseases like lupus or scleroderma
- Conditions that could interfere with wound healing and blood clotting
- A weakened immune system (such as women receiving immunosuppressive therapy)
- Reduced blood supply to the breast tissue
- Radiation to the breast following implantation

"Talking to my colleagues and looking at my own numbers, I think within the Beverly Hills area at least, most plastic surgeons are down by about 30 percent, some people even to 50 percent," said plastic surgeon Dr. Payman Simoni.

It's nothing like the car business. You don't see lines of plastic surgeons lining up on Capitol Hill looking for a handout. But there are similarities. Doctor Simoni, who has a thriving practice on La Cienega Boulvard says many patients, like car buyers, can no longer borrow money to pay for it.

"One of the problems also we're having is the financing issue. A lot of people used to finance their procedures, and now we're realizing that the finance companies are holding their money tight," said Dr. Simoni.

The rich and famous are still getting their faces lifted, but cosmetic surgeons who practice around Beverly Hills who were flying high just a couple of months ago, say business because of the economy is definitely going down.

Dr. David Alessi may be the exception. Dr. Alessi says he hasn't seen a downturn yet at his practice on Wilshire Boulevard, because he has held on to his high-end clientele.

"It seems that when times are bad, people want to take care of themselves, and one of the easiest ways to take care of themselves is to do something plastic-surgery-wise that makes them feel younger and healthier, more confident about themselves," said Dr. Alessi.

Dr. Allessi and others say because of the tough economic times, patients are opting for more Botox procedures and antioxidant creams to smooth a face without surgery. Doctor Simoni has developed techniques that allow lifts without the use of expensive anesthetics. Plastic surgeons, like everyone else, are adjusting to the new wrinkle in the economy.
